Practical Context Engineering Using LangChain for AI Developers ( A Comprehensive Guide)
I’ve never fixed a production agent by upgrading the model. Not once. Every fix came from changing what the model saw.
There is a moment every AI engineer recognizes.
The agent worked in your notebook. The model was smart, the tools ran clean, the prompts looked solid. You shipped it. Two days later, it is hallucinating tool calls, ignoring instructions, and making decisions that make no sense given what the user asked.
Your first instinct is to blame the model. Swap GPT-4o-mini for GPT-4o. Try Claude. Add ten more lines to the system prompt. None of it helps, and if you are honest, it never has.
Agents don’t fail randomly. They fail deterministically based on bad context.
The model is a prediction engine.
It predicts the most likely next token given everything in its context window. If the wrong information is in that window, or the right information is missing from it, the prediction goes wrong. Every time.
That is not a bug. That is how transformers work.
Context engineering is the discipline of controlling exactly what lands in that…
